As a recognized provider within both the Ohio Department of Developmental Disabilities (DODD) and Ohio Department of Aging (ODA) networks, Advocate Healthcare Services is committed to improving lives through compassionate, person-centered care. We support individuals with developmental disabilities as well as eligible older adults who wish to remain safely and independently in their homes and communities.

  • Commitment to Person-Centered Care: Advocate Healthcare Services firmly believes in a person-centered approach to care. Their services are tailored to meet the unique needs, preferences, and goals of each individual with developmental disabilities and older adults. By prioritizing person-centered care, we aim to empower individuals to live fulfilling lives while promoting their independence and self-advocacy.
  • Comprehensive Developmental Disability Services: As a provider within the Ohio DODD network, Advocate Healthcare Services offers a wide range of services designed to address various aspects of developmental disabilities. These services include:
    • Residential Care: Advocate Healthcare Services provides safe, comfortable, and supportive residential options for individuals with developmental disabilities. We create a home-like environment that encourages personal growth and community integration.
    • Personalized Support: Advocate Healthcare Services offers individualized support services tailored to the unique needs of each person. This may include assistance with daily living activities, skill development, and community engagement.
    • Community Integration: Advocate Healthcare Services promotes active community participation and social integration for individuals with developmental disabilities. We facilitate access to community resources, recreational activities, and social connections to enhance the overall quality of life.
  • Ohio Department of Aging (ODA) PASSPORT Services Advocate Healthcare Services proudly supports eligible older adults through Ohio’s PASSPORT Program. PASSPORT is a Medicaid waiver program that helps individuals age 60 and older receive long-term care and supportive services at home rather than in a nursing facility.

    Services may include:

    • Personal care assistance
    • Homemaker services
    • Home-delivered meals
    • Transportation assistance
    • Community support services
    • Care coordination
    • Respite services when applicable

Advocate Healthcare Services intends to ensure that the individuals they serve feel at home and that their care has a positive impact on their health and well-being. We encourage the participation of residents’ families in their treatment and day–to–day activities. We strive to offer a community that promotes the residents’ choices, self-respect, input, and independence, with the outcome of improving the quality of life for all of our residents.
